Training Agreement Form
This needs to be completed in full for the trainee to be enrolled in required training programme.
Please use this checklist to ensure that all the information has been completely entered on the form.
Training Agreement Change forms
If there is a change to the training through staff leaving the business, transferring to another site, withdrawing from the training at their own or the company’s request, or if the trainees details need to be corrected or updated, please use this Alteration Advice form.
If an individual trainees wishes to change the unit standards they are studying, please have them complete the Individual Trainee Unit Standard Change Form.
If you have multiple employees/trainees who wish to change the unit standards they are studying, please have them complete the Multiple Trainee Unit Standard Change Form.
Use the Multiple Trainee Qualification Add Form to expand a trainee or trainee’s current training plans
If multiple employees/trainees wish to change the qualification they are studying, please complete the Multiple Trainee Qualification Change Form
